Please note that all fields of this section are compulsory.

  3. Write the Reference Number.

Section B: Personal Information

  2. Date of birth.
  3. South African identity number.
  4. Passport Number (If you are not a citizen of South Africa).
  5. Select the race.
  6. Tick the gender and mark what is your gender.
  7. If you are not a South African citizen, please specify your nationality in the next field.
  8. In case you are not a South African citizen, please specify that you have a valid work permit.
  9.  Do you currently have any unresolved criminal cases pending against you? If so, please provide the details.
  10. Have you ever faced dismissal from the Public Service due to misconduct? Please select the options. 
  11. Do you currently have any pending disciplinary cases against you? If so, please provide the details.
  12. Write the total number of experience you have.

Section C: Contact details and medium of communication.

Submit your contact details and the best way to communicate with you. There are four options: post, email, fax, and telephone. Please select one option and provide some more details about it. For example, if you have selected the email, then write your complete email in the next field.

Section D: South African official language

This section contains two fields: the number one is speak, and the second one is write or read. State ‘good, “fair,” or ‘poor in the respective fields.

Section E: Formal Qualification

Name of school and qualification obtained and the year in which you completed the degree.

Section F: Work Experience

If you are already working on other jobs or have recently worked on some posts, please write in the fields.

Section G: Reference 

Write the reference in the next field and also with the telephone number.
